Qutecom is a cross platform SIP VOIP phone and IM client for Mac OS X, Linux and Windows. It is not native Cocoa but based on Qt 4.x and Pidgin, which Adium is based on. The price for calling is cheaper than Skype, and has especially good price to call Europe, as low as 0.008 €. Starting from 2.1.0 Qutecom can be used as a standard SIP client, which gives users the freedom to choose their own SIP service provider.

Apparently it can do video calls too (based on ffmpeg).
What is good also is that, as the description says, you can use any SIP provider available on the market, unlike proprietary software such as Skype and others.
